COVID-19 Vaccine Pre-Registration
This page is not intended for the general public to pre-register for a COVID-19 vaccination.
The purpose of this pre-registration is to allow workplaces, health care providers, and specific cohort settings (e.g. shelters) to register individuals who meet the current COVID-19 vaccine eligibility in our region.
Visit swpublichealth.ca/eligibility to determine who is currently eligible.
Please be advised:
- Not all individuals in Phase 2 will become eligible at the same time. Prioritization follows Ontario's Guidance for Prioritization of Phase 2 Populations for COVID-19 Vaccination.
- Regional eligibility decisions are made by the COVID-19 Vaccine Prioritization Advisory Committee. For a list of groups currently eligible to receive the vaccine please visit our COVID-19 Vaccine Eligibility page.
Cohort: Health-Related Condition & Their Primary Essential Caregiver*
Individuals who have a health condition should NOT complete this form. Only those who provide medical care for the individual (e.g. primary care provider or specialist) should be filling out this form on behalf of the defined eligible cohort.
*For clarity, if the individual with the eligible health condition cannot receive the vaccine (e.g. under the age of 16), the primary essential caregiver is still eligible.
Cohort: Residents, Primary Essential Caregivers* and Staff of High-Risk Congregate Living Settings
Individuals in a high-risk congregate setting should NOT complete this form. Only leadership at a congregate setting (e.g. Owner/Operator, CEO, Lead Administrator) should be filling out this form on behalf of the defined eligible cohort.
CONGREGATE SETTING REGISTRATION
Cohort: Essential Workers Who Cannot Work From Home
This is a pre-registration form. Individuals in this cohort are not yet eligible to book appointments.
Individual workers should NOT complete this form. Only leadership at an eligible workplace (e.g. Owner/Operator, CEO, Lead Administrator) should be filling out this form on behalf of the defined eligible cohort. Only workplaces identified as an essential workplace in Ontario's Guidance for Prioritization can pre-register.
