Farmers' Markets

Anyone that plans to open a farmers’ market and provide food to the public must submit the required information online or download, fill out, and submit the form to Southwestern Public Health.

Farmers’ Market organizers must:
  1. Notify Southwestern Public Health of the market using the Farmers’ Market Organizer Notification Form. This form must be submitted at least 14 days prior to the event. 
  2. The organizer is responsible to ensure that each food vendor at the market submits an individual Farmers’ Market Vendor Notification Form prior to selling ANY food items at this location. This form must be submitted to Southwestern Public Health at least 14 days prior to the event.

If you have any questions with the Farmers’ Market notification process, please call 1-800-922-0096. A Public Health Inspector will review the forms and if there are any questions or concerns, they will contact the person responsible.
