Ontario's COVID-19 vaccine program has transitioned to an annual program that aims to ensure Ontarians are protected against COVID-19 disease and severe outcomes such as hospitalization and death.
Eligibility
- All individuals six months of age and older who live, work, or go to school in Ontario are eligible to receive one dose of COVID-19 vaccine each fall
- Children six months to four years old and those with certain medical conditions may require additional doses of COVID-19 vaccine during their first season being immunized
- Specific populations at high risk for infection may be eligible for a dose in the spring (April to June). Individuals who are not included in the specific high-risk populations can begin or complete their primary series in the upcoming fall season

- At this time, fall clinics have ended
- If your child meets the criteria for “high risk” and requires a dose in the spring, please have their specialist contact us

COVID-19 vaccination records
Southwestern Public Health (SWPH) can only access COVID-19 vaccination doses administered between October 2025 and March 2026. To request your vaccination record for doses in this timeframe, please complete the request form here. If you received a dose outside of this timeframe, it will not appear in an SWPH record request.
The Ontario Ministry of Health has advised that records for doses given before October 2025 are not expected to be available until fall 2026 (exact date to be confirmed). At that time, COVID-19 vaccine records are expected to be available through SWPH’s Immunization Connect Ontario (ICON) portal.
