Please note, this page is NOT for booking COVID-19 vaccination appointments. To book a COVID-19 vaccine at any of our locations, book online at covid-19.ontario.ca/book-vaccine OR call 1-833-943-3900

Southwestern Public Health provides immunizations by appointment. You will need a separate appointment for each person being immunized.

 Please review the following information before booking an appointment:

  • A health card may be helpful to find your record, but it is not required at our clinics
  • Children who are too young to make informed health care decisions must be accompanied by or have written consent from a parent or legal guardian
  • Bring your immunization record with you if possible

TB skin testing cannot be booked online.
