Parents and caregivers need to report all of their children’s immunization to Public Health. You can report immunizations in person at one of our locations, by mail, or online. If you are over 18 years old, you do not need to submit immunization information.

Report out of province/country COVID-19 vaccination

If you do NOT live in Elgin County, Oxford County, or the City of St. Thomas, please contact your local health unit to register your out-of-province/country COVID-19 vaccination.

Why do I need to report immunizations?

Health care providers do not inform public health when they provide needles. You need to report immunizations so we can help keep a record for you. You need to show an immunization record to:

  • Register for school in Ontario
  • Register for child care
  • Attend camps or other children’s programs
  • Work or volunteer in some jobs
  • Travel to some countries

How can I get a copy of my immunization record?
Immunization records that have been reported to public health are available online:
  • View immunization record if you live in Elgin County
  • View immunization record if you live in Oxford County

If you can’t find your immunization record online, contact the office where you received any needles and ask for a record of what they gave you. If you have provided your local health unit with a record in the past and aren’t able to find it online, contact us for a copy.

Why can’t I find my immunization record online?
Your immunization record is not available online if:
  • Your immunization record has not been reported to public health; or
  • Your health card number has not been provided to a health unit and/or entered into the immunization database
Can you help with medical paperwork for a school placement or job?
Unfortunately, Southwestern Public Health is not able to complete medical paperwork for school placements or jobs. Please make an appointment with your health care provider.
Can I get vaccines for school or work at Southwestern Public Health?
If you know which vaccine you need, call us to discuss your options. Some vaccines may be available for free while others must be purchased and administered at a pharmacy or through your health care provider’s office.
