Healthy Babies, Healthy Children is a free province-wide program for families who are pregnant or with young children up to school-age. The HBHC program is free, voluntary and you do not need an OHIP card in order to receive service.

What is this program?

Through this program, a Public Health Nurse and/or Parent Resource Worker provides information, support and referrals to parents and families. This might include a phone call after you have had a baby, and/or home visits for additional supports. 

It is a free home-visiting program that supports you and your family with:

  • having a healthy pregnancy
  • developing a positive relationship with your child
  • promoting your child’s growth and development
  • connecting you to resources and programs within your community
  • working together to give your child the best start in life

Eligibility

This program is offered to pregnant people and their families:

  • When they are pregnant
  • After the birth of a child
  • Until their children start school

Submit a referral form 

HBHC referrals can be made as a self-referral or a professional referral from a health care provider, such as your family doctor, midwife, nurse, or OBGYN.
