How to report a possible rabies exposure
If a person is bitten or scratched by an animal, the incident should be reported to Southwestern Public Health (SWPH) so that a Public Health Inspector (PHI) can investigate and ensure the victim was not exposed to rabies.
To report an animal bite, please complete the possible rabies exposure report form and fax it to 519-539-5059.
